Get a list of those with whom we’ve shared information
For the six years prior to the date of your request, you are entitled to a list (accounting) of the occasions on which we disclosed your health information, with whom, and for what reasons.
We will include all the disclosures except for those about treatment and health care operations, and certain other disclosures (such as any you asked us to make). We’ll provide one accounting a year for free but will charge a reasonable, cost-based fee if you ask for another one within 12 months.

Choose someone to act for you
If you have given someone medical power of attorney or if someone is your legal guardian, they have authority to act on your behalf and make decisions regarding your health information.

We will not retaliate against you for filing a complaint.

We use your health information and share it with other medical experts who are providing you with care.
Example: A physician who is treating you for an injury asks a different physician about your general health.

We use and share your health information to run our practice, improve your care, and contact you when necessary.
As an illustration, we utilize your health information to coordinate your care and services.
Promotion and Advertising
